GOOGLE

• Click on the link(s) below to open a new Google maps window.
• Sign in to your Google account or create one.
• If you do not have a gmail account, please click here and follow the instructions.
• Scroll down to “Review summary” in the left-hand column and click the “Write a review” link.
• In the window that appears, click the stars to provide a score. You can also write a review.
• When you are finished, click on the “Post” button.

HEALTHGRADES

• Visit www.healthgrades.com/doctor-reviews.
• Type in the name of your doctor/provider.
• If they are in the Healthgrades system, you will be taken to the doctor’s review page.
• Scroll down to the reviews section and select a star rating on the box that says “Leave a review.”
• A review page/pop-up window will appear where you can leave details of your review.
• When you are finished, click the “Submit” button.

YELP

• Visit www.yelp.com/writeareview.
• Type in the name and location of your doctor/provider.
• Their name and/or their practice name should appear.
• Select the “Write a Review” button.
• Click a star rating and type your feedback/review in the text box.
• Click on the “Post Review” button.

WebMD

• Visit www.webmd.com.
• Click on Find a Doctor at the top of the page.
• Enter the provider's name or practice and location.
• Once results appear, click on the provider's name.
• Click on “Rate this Doctor” at the top of the page.
• On the review page that appears, click the stars to provide a score. You can also write a review.
• Click on the “Submit Ratings” button.

VITALS

• Visit www.vitals.com.
• Search for your provider by name, specialty or condition, and specify the location.
• If they are in the Vitals system, the provider profile will appear on the search results page.
• Click on the provider’s name or the “View Profile” button for the provider.
• Click on “Write A Review” in the “Rating Overview” box.
• Select a star rating and enter your feedback/review in the text box.
• Click on the “Submit” button.
